The 2015 Annual Verification forms were mailed with the October benefit checks/ direct deposit notices. Retirees/ survivors who have not mailed/ brought their forms in have until February 19th to return the form to us. If not returned it will result in your February benefit being held until the verification form is received. If brought into the office, in person, it does not need to be notarized, but you may be asked to produce a photo I.D.
