2017 Retiree Annual Verification Form


The 2017 Annual Verification forms were mailed with the October benefit checks/ direct deposit notices. Retirees/ survivors who have not mailed/ brought their forms in have until December 4th to return the form to us. If not returned it may result in your benefit being held until the verification form is received. If brought into the office, in person, it does not need to be notarized, but you may be asked to produce a photo I.D.

Annually every retiree and survivor receiving monthly benefits will receive an "Annual Verification Statement" form. Each retiree/ survivor must complete, sign and return this form to our offices promptly. Failure to return the appropriately signed "Annual Verification Statement" will result in benefits being held.
